As a Spiritual & Religious Care volunteer or clergy with visiting privileges, I acknowledge that:

  • I will maintain current knowledge of my role and emergency code procedures
  • I will participate in required training and evaluation activities
  • I must remain in good standing with a recognized faith group and provide proof of endorsement upon request
  • I am aware that I am entering an area of service with the expressed permission of the Spiritual Care Department
  • In the event that my voluntter involvement is not compatible with the hospital rquirements or or contravenes the spirit of the guidelines for offering Spiritual Care at Southlake, the decision of the Head of Spiritual Care will be final (i.e. retraining or termination).
  • I hereby grant permission for my personal contact information (phone number and email) to be shared with my placement supervisor and other volunteers in my work area for the purposes of scheduling and relaying information.

  • The personal information collected in this application will be used as part of the application screening process to evaluate my suitability for a volunteer position at Southlake Health.
